There was a ton of red in this episode. The gigantic red flower by Sun's garden...the red tomato by the reddish campfire...the red lamp and red swirls in the pillows in the hotel...and of course Sun's red blood (several times) and I think she was wearing a reddish type of dress at the end.

I thought of jack or Ethan....but you're right she's pregnant so it will be more JUliet (or Ethan!!!)
But my bet is on JUliet based on previous seasons...Sun was a patient!
Claire was Ethan's patient to it makes perfect sense!

She will not die because Juliet will save the baby I think everyone's right about that. Besides, Hurley wore a red shirt for almost all season 4. Those rules have been changed.
