we know juliet does not want to be on the island. she's repeatedly asked to leave but ben has denied her each time. she was supposed to leave with jack but since locke blew up the only way off the island (assuming the sub WAS the only was off the island), she's now trapped there.
we know ben was using her and making her do things she wouldn't normally do, like kill pickett, by dangling the opportunity to leave the island in front of her like a carrot on a stick. we also know she is a fertility doctor and was brought to the island to remedy a very specific problem; that women who conceive on the island always die before giving birth. up to this point, it appears as though her research has done nothing to fix the problem and women are still dying.
interestingly, what we also know is that juliet was sleeping with goodwin, and tom's comment regarding juliet and ben ("they've got history..."), seems to imply that there may have been a romantic relationship between the two. either way, juliet has had sex on the island.
now that she can't get off the island, it's strange that she's still doing ben's bidding. her sister had her child and was cured of her cancer (if it ever came back in the first place) so ben seemingly has no way of manipulating juliet to continue her work on pregnant women...
unless juliet is now personally involved in the very problem she was meant to fix. if she knows she's going to die, she has that much more reason to continue her research and find a cure soon. this would explain why she's still eager to help the man she claims to hate, and maybe give us insight into why juliet always seems to be wearing very loose-fitting clothes...
