Jack's tattoos feature the number 5 which is surrounded by tiny marks, much like the seconds on a clock. Could his tattoo symbolize the 5 seconds of fear he gives himself in a tense situation?
I tried a search too because I thought I'd seen that theory here before, but then I remembered that it's my friend's theory from work. It's entirely possible in the context of the character, though that is actually Matthew Fox's tattoo also so if there is any relevance to it on Lost, it might feel somewhat forced.

If it is really his tattoo they deliberately erased it from the flashback scenes in Man of Science Man of Faith, Specifically look at the scene where he is with Sarah after the operation. His shirt sleeve is short enough to show the tattoo if it was there. Also, he has tattoos on the inside of his left arm which can't be seen either.
Of course, this is just speculation, but I heard that Jack's tattoo represents '5 B.C.' that being the year that Jesus Christ was thought to be born in. And if you consider all the other biblical references in the show, it does make since. Jack's last name is Shepard, as in 'The Good Shepard.' Oh, and he was regarded as a miracle worker or something like that by the girl he kissed.
